Because the HLA system has such a dominant role in transplant immunity, pretransplant histocompatibility testing is important. The practical problems remain as to whether tissue typing can be applied prospectively for all types of transplants. Organ preservation time remains an important limitation for an HLA-based tissue allocation strategy, especially of heart, lung and intestines. Nevertheless, histocompatibility testing yields relevant information for clinical management of any type of transplantation.
