TRANSITION FROM PEDIATRIC TO ADULT CARE
Planning for transition begins at an early age and is focused on helping patients to develop into independent and confident young adults capable of caring for their chronic condition. Support is provided to caregivers as their roles change with their child's evolving needs.
This planning involves good communication between the patient and family and the pediatric and adult transplant providers. The goal is to help patients develop the knowledge and skills they need to manage their own care and make good personal and medical decisions, for example to take their medications as prescribed or follow up with clinic and test appointments as recommended. During this time, the doctors and nurses will talk about a variety topics, including signs and symptoms of infection and rejection to insurance and pharmacy issues
