Recurrent autoimmune disease is another potential problem. Although end-stage organ failure due to autoimmune disease can be successfully treated with transplantation, HLA-restricted T-cell autoimmunity can be expected to persist in the patient. This may promote recurrent disease, especially if the donor organ shares the relevant HLA antigens with the recipient. Thus HLA compatibility can under certain conditions promote certain nonalloimmune mechanism of allograft injury.
